Hi all,
Newbie here.. Be nice!
I'm running Debian Etch 2.6.18-6-xen-686 and am getting error 404 when
I run apt-get update.
Am I right in thinking that I need to update my /etc/apt/sources.list
file? What are the correct, Bitfolk cached entries?

Debian Etch is no longer a supported release, and are hence no longer
available from regular sites such as http://ftp.uk.debian.org and
http://security.debian.org. While you really should upgrade there is
still the option of using http://archive.debian.org/ in the meantime.
Do note http://www.bitfolk.com/apt-cacher.html and the text: "Only the
specific mirrors listed above are allowed. If you use other sites in
your sources.list then please let support know and we will consider
whether to add them.". Of course, there is still the option of using
archive.debian.org directly, without going through the apt-cacher.

You might be out of luck using the bitfolk apt-cacher, as it looks as
if etch has been removed from ftp.uk.d.o (and has been for some time?)
ftp://ftp.uk.debian.org/debian/dists/
...so you may have to use an archive mirror which has etch (oldstable)
available. Furthermore, security support for etch has been
discontinued, so I strongly urge you to upgrade to lenny ASAP.

So can you clarify.. you've done an upgrade between etch and lenny
but haven't yet rebooted?

I've done etch->lenny many times on BitFolk VPSes and apart from the
above there haven't been any issues. You just follow the regular
upgrade instructions from the release notes of lenny.
But still, when it comes to major distribution upgrades we always
advise that customers ask for a snapshot to be made of their block
devices. If things go terribly wrong then we can fairly quickly
restore that, as opposed to having to do a clean install.
There is also the option of asking for a new VPS to migrate into:

From the way the bug counter is going, I think they're aiming for a 1st
