I've not had major problems upgrading either myself. My current server started
life on 9.04 (because of software versions I wanted) and then went through 9.10
to 10.04 where it has stayed pending the next LTS. I'm about to rebuild the
hardware so it will likely be reinstalled with 10.04 and then upgraded when
12.04 is released. I think this desktop started life as 8.04 and has upgraded
with each release, but I could be wrong. The one thing I have noticed in favour
of a reinstall is picking up on some of the tweaks and policy changes that you
don't get if you upgrade a configuration - although that is less of an issue
with a server; well a proper one with no GUI anyway ;)
That said upgrading in a more gradual fashion by migrating to a new VPS does
have its attractions. I'll have to see when I've got two weeks where I can
ensure the time to concentrate on it for long enough - oh and decide what I
plan to do when 12.04 comes out, I'm quite keen to try out the new version of
Dovecot that will come with it.

etch->lenny->squeeze in sequence
without a reboot until the very end, but that was on a simple
single-purpose VPS and to be quite honest I trust Debian's
in-place upgrades a lot more.
As a long time Debian user I agree that upgrading Debian is normally
straightforward but I wouldn't dream of trying to go straight from
etch > squeeze in one step, it needs to be done in stages as Andy says.
From what I've read Ubuntu seems to need re-installing in order to
upgrade between versions safely.
